In the event of a medical emergency I authorize LGCS/Knowledge Quest staff to administer First Aid, CPR, and/or to transport my child to the nearest hospital.

I give permission for my child to be photographed during LGCS Summer Camp. I understand that my child's picture may appear on Los Gatos Christian School's or Knowledge Quest's website and/or may be used for future Los Gatos Christian School or Knowledge Quest advertising.

I do not give permission for my child to be photographed during LGCS Summer Camps.

CANCELLATIONS & REFUNDS POLICY

If the camp you register for is canceled by us, we will help you select another camp. If another selection will not work, you will receive a full refund (less the credit card processing fee). For cancellation 14 days or more before the first day of camp, all fees paid less $100.00 will be returned, or full payment transferred to another camp, if possible. If the cancellation is less than 14 days before the first day of camp, no refund will be made. In the event of illness, accident, or family emergency, all fees LESS A $25.00 HANDLING CHARGE will be returned if notice is given prior to the first day of camp. No refunds or fee transfers will be made for those who fail to show up on the first day of camp.
